This is a theme for Team Win Recovery Project for the Galaxy Nexus (or any 1280×720 device). You must have TWRP installed which is available HERE.
Download the attached file and place it in /sdcard/TWRP/theme (If the theme directory does not exist. create it. Case sensitive.)
I found the button images online while searching for an xml to do this theme, it had no author or creator so credit to whoever that guy is.
